How to gain access to your patient portal:
1. Make an appointment with Dr. Hubbard
2. Upon check in, we will either review your current paperwork or ask that you update our
forms. When doing so make sure to list your email address. By giving us this
information, your login credentials and instructions will be sent directly to you.
3. No Email? No Problem!
If no email is provided our system will automatically generate a unique username and
password for you. You will be handed the login credentials and instructions while you
are in our office, giving you complete access to your portal.
